Advanced Practice Consultant - Wound, Ostomy and Continence

As a member of the Knowledge and Practice Portfolio at Saint Elizabeth Health Care (SEHC), you will promote excellence in evidence based wound, ostomy and continence therapies for clients in the community setting. You will lead wound, ostomy and continence programs and support other Corporate initiatives, including setting practice standards, defining clinical processes and procedures, program development, and quality/risk management.
Responsibilities
- Evaluate wound, ostomy and continence programs and services using performance indicators and clinical outcome measures.
- Lead program development of wound, ostomy and continence services.
- Promote wound, ostomy and continence services within the community and establish partnerships and links with key stakeholders.
- Lead and/or contribute to proposal development and/or RFP responses for wound, ostomy and continence programs and services.
- Support orientation and preceptorship of Wound Ostomy Resource Nurses (WORN)/Enterostomal Therapist (ET) new hires.
- Develop educational initiatives and support the ongoing development of our innovative on-line learning environment.
- Assist Clinical Resource Nurses and WORNs/ETs with problem solving clinical issues.
- Support and manage the virtual ET consults as well as the process and program as content expert.
Requirements
- Master’s degree in Nursing or a related field and certified ET designation (candidates eligible for and willing to obtain the designation as a condition of employment will be considered).
- Registration with the College of Nurses of Ontario and membership within the CAET.
- A minimum of three years of community nursing experience.
- An understanding of principles of adult education.
- Advanced knowledge of program development and evaluation.
- An ability to promote and facilitate evidence based practice within a community setting.
- An ability to work under minimal direction with demonstrated initiative and ability to be self-directed as well as an effective team player.
- An ability to be resilient within a changing environment.
- A demonstrated commitment to continuous learning and improvement.
- A vehicle, travel, and some on-call are required.
